The following information is intended to provide a broad definition of the term, Plan, and other terms used in conjunction with it, in relation to its meaning to business practices.

Plan:

• A contract between an insurer and a group or client to provide benefits.

Or

• An active member of Delta Dental Plans Association or an entity wholly owned or under the sole control of such a member, e.g., Delta Dental Plan of Ohio.

Plan Name — name of the member of Delta Dental Plans Association for which the client has a contract to obtain benefits from. For example, Delta Dental Plan of Michigan (DDPMI).
