Manage Enrollments
The Enrollments screen displays all of the information associated with the subscriber. To access the screen, select the Manage Enrollment
menu option. The displays:
- Subscriber — the name of the subscriber
- SSN — the Social Security Number associated with the subscriber
- Account Number — the account number assigned to the subscriber
- Subscriber Information:
- First Name — the first name of the subscriber
- MI — the middle initial of the subscriber
- Last Name — the last name of the subscriber
- Birth Date — the date of birth provided for the subscriber
- Gender — the sex of the subscriber
- Branch of Service — the branch of service (Marines, Air Force, Navy, Army,etc) the subscriber served in
- Spouse Information:
- First Name — the first name of the subscriber's spouse
- MI — the middle initial of the subscriber's spouse
- Last Name — the last name of the subscriber's spouse
- Birth Date — the date of birth provided for the subscriber's spouse
- Gender — the sex of the subscriber's spouse
- Dependent:
- First Name — the first name of the subscriber's child(ren)
- MI — the middle initial of the child
- Last Name — the last name of the child
- Birth Date — the date of birth provided for the child
- Contact Information:
- General Address Information:
- Address Line 1 — the street address where the subscriber has requested information to be sent to
- Address Line 2 — the apartment or PO Box number of the subscriber
- City — the city where the address exists
- State — the state where the subscriber lives
- Zip Code — the postal code for the address provided
- Phone (Home) — the home number of the subscriber
- Phone (Alt) — the an alternative phone number for the subscriber such as a cell phone number
- Email Address — the email address of the subscriber

How to submit your queries or questions online
- At the bottom of the Payment Account Statement screen there is a statement about submitting your questions or queries. Select the link.
- The Privacy Act Statement window opens. After reading the statement, select the I have read the Privacy Act Statement button.
- Another window opens the Online Customer Service Inquiry Form screen. Enter all pertinent information and select Submit.
- A message appears confirming the inquiry was received. You will receive an email response acknowledging your submission.
